Defunct processes in keepalive mode.

This sounds like a problem with the child clean up code specific to keepalive. I'll look into this as soon as I get some free time.

I am unable to reproduce this. I'll leave the issue open indefinitely as it looks legit. If anyone can reproduce this reliably, please post a comment here with specifics and I'll look into it.
